When you come for your appointments, please remember to bring the following:
- A valid driver’s license / ID card
- Current insurance ID card
- Any referral letter (if required)
- Any prior imaging studies ( x-rays, MRI, CT scan ). Please bring the actual images (films or CD) and any reports.
- List of any current medications
